General Practitioner Referrals
We work with General Practitioners to provide optimal health care for our patients. RehWork Exercise Physiologists specialise in exercise prescription, with a strong focus on self-management. You may refer patients privately (with the option of using an allied health care plan) or under the Workers Compensation system for McKenzie assessment and treatment, functional activity programs implemented with a cognitive behavioural approach for persisting pain patients, muscloskeletal rehabilitation for back, neck and shoulder pain and post operative rehabilitation.
Once you've completed the referral form below you'll see a message to confirm it has been successful and receive an email from us with a copy of the information for your records. We'll then be in contact to discuss the referral.
