General Practitioner Referrals
We work with General Practitioners to provide optimal health care for our patients. RehWork Exercise Physiologists specialise in musculoskeletal rehabilitation, with a strong focus on self-management and coping. You can refer private patients (with the option of using an allied health care plan) or patients insured under the NSW Workers Compensation scheme for assessment and/or treatment. We implement a biopsychosocial treatment approach for conditions including musculoskeletal injury, post operative rehabilitation and chronic pain.
Once you've completed the referral form below you'll see a message to confirm it has been successful and receive an email from us with a copy of the information for your records. We'll then be in contact to discuss the referral.
